The Stock ph on the fmg4 is not the actual lonex pom. Its made out of a cheaper plastic and doesnt have as many ports. There is also a hard plastic washer to try and correct the aoe behind the piston head. The pad on the cylinder head is extremely hard as well. The first time I felt it, I knew it was a matter of time before it broke. I was well aware of this, as I replaced it with a true Lonex POM and then corrected the AoE with sorbothane. I think that, like you said, it was a lesser-quality plastic with no padding that was likely to fail after a decent amount of use. I just wanted to make sure it didn't indicate any other possible issue. The only other possibility would be that it gave a little to much force on the piston, reducing the overall strength. However the lonex red is extremely strong, and you said that you halted fire almost immediately.

Personally I like to use metal piston heads as it is continuously taking force, there for less likely to break. Ill take more reliability and sacrifice weight any day over speed, and metal piston heads don't slow the rof and trigger response down that much, or at least in my setup it dosent.

I know that some people do use metal piston heads. From what I've seen, the vast majority don't. With a sorbo buffer it's likely unnecessary, as I think the lack of a sorbo is what caused this failure.
